GLP-1 Medication Before Surgery: Endoscopy and Anesthesia

Tell the procedure team about GLP-1-based treatment when the appointment is arranged. A universal “stop for one week” instruction does not fit every patient. Delayed stomach emptying can matter during general anesthesia or deep sedation because stomach contents may enter the lungs. [1]

Current US multisociety guidance allows many patients to continue, while higher-risk circumstances require an individualized plan. Local protocols and the procedure itself still matter.

Fasting does not replace disclosure

Following routine fasting instructions is essential, but it does not remove the need to disclose the medicine. The team may consider a modified diet, assessment of stomach contents, a different anesthetic approach or postponement in selected circumstances. [1] Tell them about recent nausea or vomiting even if you have already withheld a dose. Do not use your lack of symptoms to decide that disclosure is unnecessary.

Get one coordinated written plan

If instructions from the prescriber and procedural clinic differ, ask them to reconcile the plan before the day of treatment. Write down the last planned dose, permitted food and fluids, management of diabetes medicines and the restart instructions. Do not choose an interval from an online checklist or extend fasting yourself. The decision must balance procedure safety with the consequences of interrupting treatment.

Emergency care should not wait for a washout

In an emergency, share the product and last-dose information as soon as possible. Do not delay attending because you think the medicine needs time to leave your body. The anesthetic team can assess the urgent situation and take appropriate precautions. Afterward, confirm when and how to resume, especially if eating remains difficult or several doses have been missed.

Frequently asked questions

Does a sedated endoscopy count?

Yes. Deep sedation can make the issue relevant; tell the endoscopy team.

Should everyone follow a liquid-only diet?

No. Use the specific instructions given for your risk and procedure.

Can I restart as soon as I get home?

Confirm the plan with the treating team rather than assuming your prior schedule still applies.
