GLP-1 Medication and Pregnancy: Planning and Breastfeeding
Weight-loss treatment with these medicines is not appropriate during pregnancy. If pregnancy is recognized during weight-management treatment, stop the medicine and contact the prescriber and maternity team promptly. Planning ahead allows time to discuss discontinuation and any alternative care. [1,2]
Breastfeeding requires a separate assessment. Ingredient, formulation, infant circumstances and the reason for treatment all matter.

Do not transfer one product rule to another
US Wegovy labeling advises stopping at least two months before a planned pregnancy. Zepbound labeling advises a non-oral contraceptive method or added barrier protection for four weeks after starting and after every dose increase when oral hormonal contraception is used. [1,2] These are product-specific instructions, not one rule for all weight-loss medicines. Confirm the applicable local label and your own plan.
An exposure is a reason for consultation
If you used the medicine before knowing you were pregnant, record the product, dose, last use and estimated gestation. Exposure alone does not establish an individual pregnancy outcome. Avoid making decisions from isolated stories online. The maternity and prescribing teams can provide an exposure assessment and follow-up appropriate to your situation. [3] If treatment was also supporting diabetes care, address that need promptly rather than leaving a gap.
Breastfeeding evidence is not interchangeable
The US Wegovy label does not recommend breastfeeding during tablet treatment; oral and injected formulations should not be treated as identical. Tirzepatide milk-transfer information also does not answer every question about long-term infant outcomes. [1,2] Discuss the infant’s age and health, your treatment needs and feeding goals. Do not restart a pre-pregnancy dose without a new review after a long interruption.
Frequently asked questions
Can I just avoid dosing around ovulation?
That does not provide a reliable safety plan. Discuss the necessary advance discontinuation for the product.
Does one unintentional dose mean harm is certain?
No. Stop further weight-loss treatment and obtain individualized exposure advice.
Can I restart immediately after delivery?
Not automatically. Breastfeeding, current health and the interruption need reassessment.
