HOMA-IR Insulin Resistance Calculator

Calculate the HOMA1-IR index from fasting glucose and fasting insulin drawn at the same time. The number is not a diagnosis or individual treatment threshold.

μIU/mL and mIU/L have the same numeric value. Do not enter pmol/L without an assay-specific conversion.

How to use this tool

For fasting glucose 90 mg/dL and insulin 10 μIU/mL from the same draw: 10 × (90/18) ÷ 22.5 = 2.22. Choosing 5.0 mmol/L gives the same value. This example is not a clinical threshold.

Understanding the result

A higher value may reflect greater insulin resistance than a lower value in a comparable population. There is no universal diagnostic cutoff or low/high risk band. This number alone does not diagnose insulin resistance, diabetes or PCOS.

Limits

This indirect index assumes a fasting steady state. Insulin assays vary, and insulin treatment, impaired insulin secretion, acute illness or pregnancy can make comparisons especially difficult. HOMA1 and HOMA2 are different models.

Frequently asked questions

Does a value over 2.5 diagnose insulin resistance?

No. Research cutoffs vary by population, assay and study outcome. This tool does not assign a universal normal or abnormal label.

What if I do not have an insulin result?

HOMA-IR cannot be calculated from glucose alone. Discuss standard tests such as fasting glucose and A1C with a clinician.
