Requesting an English Lab Report in Korea: A Bilingual Checklist

KOREAN HEALTHCARE PAPERWORK

Ask the receiving institution first, then the hospital in Korea

Before requesting an English version of test results from Korea, confirm the document types, date range, language and format required. Even within one hospital, English-language availability, fees, processing time and collection options can vary by document and request channel.

Before you apply

  1. Ask the receiving institution first. Confirm document types, date ranges, whether Korean originals and English translations are both needed, required forms or file formats, and the deadline. Ask separately about translator qualifications, translation certification, notarization or an apostille if relevant.
  2. Confirm the scope with the issuing hospital in Korea. Identify test reports, certificates, physician statements, record copies and imaging materials by their exact names. An English website does not establish that every record is available in English.
  3. Check requirements and the estimated total fees. Ask about the department, appointments or consultations, patient or representative documents, name spelling, and any document, consultation, translation, image-copy, download or delivery charges. Confirm when each document can be ready.
  4. Confirm official collection and submission methods. Check whether you are eligible for online, in-person or postal services. Use the receiving institution’s secure portal or approved delivery method, then confirm receipt and completeness.

Distinguish documents with similar names

Document names and contents can differ between institutions. Use these distinctions as a starting point for questions.

Test or laboratory report / 검사결과지

Lists test results and related details such as units and reference ranges. Confirm the tests, date range and availability of a full English version.

Medical certificate / 진단서

A certificate prepared by a doctor. Its request process may differ from a copy of a test report; ask whether a consultation is needed.

Physician’s statement / 의사 소견서

A statement giving a doctor’s opinion for a particular purpose. Ask what the receiving institution needs and what the hospital can provide.

Copy of medical records / 의무기록 사본

A copy of selected existing records. Specify the date range and record types; requesting copies does not establish English translation availability.

Discharge summary / 퇴원요약지

Summarizes an inpatient stay and information at discharge. Confirm the exact document: a certificate of admission and discharge dates serves a different purpose.

Imaging files / 영상 파일

The images themselves, such as CT or MRI images. CD/DVD is a storage medium; DICOM is a medical imaging information standard. Confirm the format and the receiving institution’s ability to open and accept it.

Imaging or radiology report / 영상 판독문

The written interpretation of an imaging study. Ask whether it is needed separately from the images and whether an English version is available.

A DICOM format or CD does not by itself guarantee that every institution can open or accept the material. Confirm the images and written report separately.

Blank bilingual questions for institutions

Ask the receiving institution questions 1, 2 and 10 first. Ask the issuing hospital in Korea questions 3–9. Copy only what you need. Print or save this blank checklist as a PDF, then complete it privately.

This checklist does not replace official request, consent or authorization forms. Do not upload the completed sheet, medical records or identity documents to this site or its comments.

If the button does not work, use your browser’s Print menu. Check the destination and pages in print preview before printing or saving as PDF.

Life Choice Note · General administrative preparation · Checked 2026-10-03

Check the documents after collection

  • Check the requested name spelling, document types and test date range.
  • Check all pages and attachments, and whether the English document or summary covers the requested scope.
  • If originals, signatures, stamps or translation certification are required, confirm that those requirements have been met.
  • If both images and a written report are required, check that you received each and have instructions for viewing the files.
  • Ask the issuing hospital to correct discrepancies or missing material. Do not alter the original record yourself.
  • After submitting through the receiving institution’s approved method, confirm receipt and whether anything else is needed.

Continue with existing preparation tools

Keep the original test name, result, unit, reference range and date together. Check whether the date refers to sample collection, testing or reporting, and discuss actual results with your clinician.

Official guidance and scope checked

Checked: 2026-10-03. These are examples of institution-specific procedures, not rankings or nationwide issuance rules. Recheck current guidance and the channel available to you when applying.

Samsung Medical Center describes different collection routes in its international outpatient and general medical record guidance. Asan Medical Center distinguishes copies of existing records from English certificates prepared by a doctor. Seoul National University Hospital also separates record/image copies from certificate procedures. Mayo Clinic directs patients to follow their care team’s instructions for submitting outside records. Do not apply one institution’s conditions to another.
